^I would contact D2D. They've made changes to some of the game files that I can't account for (Steam did the same thing), all in the name of getting the game to run on Vista/7, and it's their responsibility to get the game to run on a modern OS.

KotOR2's a real bitch sometimes. Try going back into that .ini file and changing the Width and Height values to 1280 and 768 respectively under both [Display Options] and [Graphics Options]. Also try running the game with V-sync on.
