I think Sam Jackson's case was the exception to the rule and done as a favor or whatever. This is supported by the fact that Nick Gillard wanted to have a yellow saber in Episode III and Lucas didn't let him. In the end, I agree with him. I don't like the rainbow fest we get with the EU.

The only "bad guy" who doesn't use red is Anakin, and that's because (as seen in the movie) he didn't have his new "Sith" lightsaber yet.
